Terms vary from provider to provider, so we recommend contacting your insurance company in advance to determine if your purchase qualifies. Please note: Function can provide detailed receipts that include a breakdown of costs, date of payment, date of service, and ordering clinician. Function cannot provide a Tax ID number, a medical diagnosis, or diagnostic codes.
Function is HSA/FSA eligible. Members can expect to receive an itemized receipt via email within one business day of scheduling lab testing. The receipt will include a cost breakdown, date of payment and service, and the ordering clinician. If you have not received an itemized receipt after one business day of scheduling your lab visits, please email hello@functionhealth.com, and we will ensure one is sent to you. Function does not provide a Tax ID number, medical diagnosis, or diagnostic codes. Reimbursement is not guaranteed. Please contact your FSA/HSA provider in advance to confirm the terms of reimbursement.
